Observation

At the time of the site visit there were plastic plugs that are a choking hazard inserted into an outlet with scotch tape over it. At the time of the site visit there was a large loose garbage bag and another bag accessible to the chidlren in the bathroom between the toddler and wobbler room. The covered area in between the buildings that is accessible to the chidlren has a couple of metal wire tomato plant holders that have pointed/sharp edges on them sitting on the bottom of an outdoor wood planter in that area. The playground has a caterpillar toy structure that is missing pieces and has some protruding pieces sticking out of it. At the time of the site visit building 2 hallway had several nap cots propped up against the walls in the walkway which are a falling hazard. There are toy pieces propped/stacked against the building area in the outdoor covered area. There are items stacked in the wobbler and toddler room bathroom stall areas. There are stacked crib railings and other itmes in the preschool room 2 storage area that is open and accessible to the children. There is a dresser against the wall of the toddler and wobbler bathroom blocking part of the entrance to one of the bathroom stalls that is not anchored to the wall. There is a rusted skillet in the covered area playground accessible to the children. The diaper changing tables in the wobbler and toddler bathroom as well as the infant room with chipping paint and sealant. The outside of building 1 just to the right as you go out the door toward building 2 building and to area by the corner of the chain link fence there is damage on the side of the building with the wood of the building coming off in chunks exposing different layers of the building with shredding pieces exposed and black coloring.
